Senior Services is committed to serving older adults, age 60 and over, who are especially frail and vulnerable.
Most of our senior clients do not have family or friends to advocate for them, help them solve problems, or ensure that they have adequate medical care and safe, affordable housing that is appropriate for their physical capabilities. The program provides coordinated services that are essential for older adults to live independently in their own homes, to manage health care needs, to experience emotional support and to participate in their community.
The Senior Services program collaborates with many area hospitals and home care agencies, the Chicago Department of Senior Services, and private and public agencies and organizations to ensure that the needs of older adults are appropriately met.
Continuity of Care
Continuity of case and trusted relationships are essential components to the overall well being of each senior in our care. Continuity of care is provided through regular assessment and case management provided by the Senior Services staff, giving each resident a voice in deciding what is needed for them to remain independent and healthy.
